A Knowledge Management team aims to bridge the know-do gap(Many of solutions to health problems of the poor exist, but are not applied. This is called the "know-do" gap -- the gap between what is known and what is done in practice.) in health by fostering an environment that encourages the creation, sharing, and effective application of knowledge to improve health.(WHO) .
